Investigation Committee for CityU Sports Hall Incident submits report

 

The Investigation Committee (IC) set up by City University of Hong Kong (CityU) to look into the Sports Hall incident on 20 May submitted its report to the President today (6 June).
  
The IC has examined in detail relevant material, including the tender documents of the green roof project and the contract terms and building plans, at seven meetings over the past two weeks. It has met with 13 people, including those in charge of the project, witnesses to the incident, and staff who followed up on reports that objects fell from the ceiling before the incident occurred. A site inspection has also been conducted.
 
After the investigation, the IC identified the possible contributing factors leading to the incident. The IC also discussed the roles and responsibilities of relevant internal staff and external parties to the project, and made a number of recommendations.
 
Professor Way Kuo, President of CityU, acknowledged receipt of the report, adding that the University was studying the findings and considering follow-up actions. The University will respond to the report in public on Friday, 10 June.
 
On behalf of the University, Professor Kuo thanked the IC members for their tireless efforts over the last two weeks in conducting the investigation and preparing the report. He also thanked staff members who have worked together so diligently to handle the aftermath of the incident. Their work had helped minimise disruption and enabled the resumption of normal campus operations.
 
The IC members included Professor Paul Lam Kwan-sing, Vice-President and Chief-of-Staff of CityU (Chairman, IC); Professor Chung Kwok-fai, Associate Head, Department of Civil and Environmental Engineering, The Hong Kong Polytechnic University; Professor James Lau Chi-wang, Managing Director, James Lau & Associates Ltd and Fong On Construction Ltd; Mr Dominic Pang Yat-ting, CityU Council Member; Professor Horace Ip Ho-shing, Vice-President (Student Affairs) of CityU; Professor Matthew Lee Kwok-on, Vice-President (Development and External Relations) of CityU; Mr Mak Hoi-wah, Assistant Professor, Department of Applied Social Sciences of CityU; Miss Wei Jin-jin, President, CityU Postgraduate Association; and Mr Lee Chak-hin, Acting President, CityU Students’ Union.
 
The Terms of Reference of the Investigation Committee are:
  1. To investigate the surrounding circumstances and identify the contributing factors leading to the incident of the roof collapse on 20 May 2016;
  2. To examine and review if the procedures adopted for the green roof project of the Chan Tai  Ho Multi-purpose Hall were in compliance with existing policies and regulations;
  3. To examine and identify if there were irregularities in awarding the contract and execution  process, and in the subsequent internal management and implementation of the project;
  4. To make recommendations to the University on follow-up actions, based on the  investigation findings, with a view to preventing similar incidents in the future; and
5.      To examine other matters that may be relevant to the foregoing.
