Donation Consent Form

Thank you for your submission and support.
We will contact your listed attending veterinarian via email to complete another online form (vet’s part).
Upon receiving the completed information from the attending veterinarian, we will contact you via phone to confirm acceptance.
Body collection will be arranged via ‘Goodbye Dear’ cremation company.
*Please farewell your pet now. You will NOT be able to farewell your pet after the body has been transferred to our college for teaching.
A post mortem report will be sent to the listed attending veterinarian within 4 months. Please contact your veterinarian for the results.
