The current work request form (for LA/LG, PAS, fumehood cleansing, TLD badge, pressure vessel) and consumable issue form have been adopted for more than a decade since CSE. In view of this, LSU re-designed both forms to streamline the reimbursement process, simplify administrative work, shorten processing time and reduce paper use. The new forms are attached (File 1, File 2 and File 3) for your information. One work request form is required for one service. The new forms will take effect on 1st Jan 2022.

Under the Chemical Weapons (Convention) Ordinance, Cap. 578, the HKSAR Government is required to submit a report to the Organization for the Prohibition of Chemical Weapons on Hong Kong's past activities involving Scheduled Chemicals (see Annex B). In order to enable CityU to collect the required data and report to the HKSAR Government for their necessary action, please note the following: Annual declarations and notifications are required for activities related to Scheduled Chemicals as listed in Annex C and Annex D (see Annex C & D). If your facility will, in the coming year, be engaged in any activities related to Scheduled Chemicals as listed in Annex C, you should apply for a permit from the Trades and Industry Department.

The Import and Export (General) Regulations (Cap. 60 sub. leg. A) (Amendments of Schedules 1 and 2) Order 2020 (Amendment Order) to control the import and export of mercury will take effect on 1 November 2020. Under the Amendment Order, “Mercury (CAS No. 7439-97-6)” will be subject to the respective import and export licensing control. Import and export licences will be administered by the Environmental Protection Department ("EPD") and issued by the Director of Environmental Protection ("DEP") under the delegated authority from the Director-General of Trade and Industry. The Chinese and English versions of the letter from EPD are attached.When purchasing any chemicals, please make sure the relevant laws and regulations are complied with. These include licence applications from relevant government departments to import, purchase, possess and store such chemicals.

The Occupational Safety and Health Council (OSHC) has issued the training programmes for the fourth quarter (Oct – Dec) of 2020. Interested staff may go directly to the OSHC Training Programmes webpage, or visit OSHC’s website (http://www.oshc.org.hk) and click “OSH Training” then select “Training Programme and Application” to view the course details. To facilitate course selection, a list of recommended safety courses for CityU staff members is available for reference.

The Chemical Control System (CCS) has been launched since 27 Jul 2020, and the purchase of hazardous chemicals shall be made using the Chemical Control System. A poster has been made to promote the use of CCS. It is posted on the FMO website under Safety and Health Information>Health and Safety Posters, with the title: HSP014_Be Safe Be Compliant Use the CCS_202007. Timely reporting and thorough investigation of incidents or accidents are critical to preventing repeat occurrences. An accident is an unplanned event that causes personal injury or property damage. A “near miss” is an event that could have caused personal injury, danger to health, and/or property damage or the environment. In the event of an accident, incident or exposure, the injured or exposed person's supervisor should complete and submit the “Injury / Accident / Near Miss Report”. In the case of an emergency, notify other people in the area and call the 24-hour Security Hotline at 3442 8888 or call 999.

LSU will work with University Radiation Protection Officers (URPOs) to revise the University’s policies and guidelines for Radiation Safety. A new policy/guideline regarding the requirement of medical examination for radiation workers will be established and submitted to the University for approval within this year. Please forward this message to relevant personnel engaged in radiation work in your department and ensure appropriate actions are taken to align with the latest policy development. By law (Cap 303), any person engaged in radiation work (who receives a radiation dose >6mSv per year during IA operation) or any work/process involving the handling or transport of any unsealed radioactive substance must undergo a medical examination by the Medical Panel. They must also be certified fit for such employment by the Radiation Board, with re-examinations every 14 months during continuous employment. Such individuals are classified as “radiation workers”.
