Workflow of Application for Animal License in NS

1. User completes the application form
2. User approaches NS Technical Team to obtain the departmental chop
3. NS Technical Team will send the completed Form 1 to DH and cc to the applicant
4. A letter of acknowledgement will be sent from the DH
5. After reviewing the application, DH will send email attached with the license (Form 2), Form 6 and Form 7

Points to Note

• No hard copy will be sent to applicant starting from 15 June 2022.
• Please refer to the Department of Health for the documents and details of application.
