Under the Chemical Weapons (Convention) Ordinance, Cap. 578, the HKSAR Government is required to submit a report to the Organization for the Prohibition of Chemical Weapons of Hong Kong's past activities involving Scheduled Chemicals (see Annex B).

In order to enable CityU to collect the required data and report to the HKSAR Government for their necessary action, please note the following:

1) Annual declarations and notifications are required for activities related to Scheduled Chemicals as listed in Annex C and Annex D (see Annex C & D). If your facility will, in the coming year, be engaged in any activities related to Scheduled Chemicals as listed in Annex C should apply for a permit from the Trades and Industry Department.

2) Please keep a record of all related activities (file link) that is related to Chemicals Controlled Under the Chemical Weapons (Convention) Ordinance. See detailed FMO filled sample (See sample).
